On this episode of Advances in Care , host Erin Welsh and Dr. Craig Smith, Chair of the Department of Surgery and Surgeon-in-Chief at NewYork-Presbyterian and Columbia discuss the highlights of Dr. Smith’s 40+ year career as a cardiac surgeon and how the culture of Columbia has been a catalyst for innovation in cardiac care. Dr. Smith describes the excitement of helping to pioneer the institution’s heart transplant program in the 1980s, when it was just one of only three hospitals in the country practicing heart transplantation. Dr. Smith also explains how a unique collaboration with Columbia’s cardiology team led to the first of several groundbreaking trials, called PARTNER (Placement of AoRTic TraNscatheteR Valve), which paved the way for a monumental treatment for aortic stenosis — the most common heart valve disease that is lethal if left untreated. During the trial, Dr. Smith worked closely with Dr. Martin B. Leon, Professor of Medicine at Columbia University Irving Medical Center and Chief Innovation Officer and the Director of the Cardiovascular Data Science Center for the Division of Cardiology. Their findings elevated TAVR, or transcatheter aortic valve replacement, to eventually become the gold-standard for aortic stenosis patients at all levels of illness severity and surgical risk. Today, an experienced team of specialists at Columbia treat TAVR patients with a combination of advancements including advanced replacement valve materials, three-dimensional and ECG imaging, and a personalized approach to cardiac care. Finally, Dr. Smith shares his thoughts on new frontiers of cardiac surgery, like the challenge of repairing the mitral and tricuspid valves, and the promising application of robotic surgery for complex, high-risk operations. He reflects on life after he retires from operating, and shares his observations of how NewYork-Presbyterian and Columbia have evolved in the decades since he began his residency. 1 134: Why Nervous System Regulation is Essential During Fertility Treatment, a High-Risk Pregnancy and Postpartum 40:16
This is the episode I didn’t want to do. I tried to avoid it, but I can’t. My feelings are prompted by what I’ve seen on social media posts and articles--and those have left an awful taste in my mouth. We have an epidemic in the US of healthcare providers not listening to women when they complain about their pain and what’s going on in their lives. People don’t listen, and they don’t trust what women say, but then they wonder why all these women’s health issues keep cropping up in spite of our advanced technology. I’m angry--and you should be, too. Let’s talk about it. In this episode: 99% of my clients have experienced being silenced, being ignored, or medical errors being made because they were not believed. In advocating for evidence-based solutions to women’s health issues, we can’t ignore the fact that every person’s body and experiences are different. We are only able to trust our instincts when we’re able to regulate our nervous system in scary situations. Regardless of your age, career, education, social status, or finances, YOU are the only expert on your body. Statistics are just big-picture guidelines, so you need to lead with your body and what it is telling you. In women’s health, providers understand the clinical side of data but ignore the lived-in experiences of women. We need more research, but your lived-in experiences matter way more than any study. Find a provider who respects your experiences and will let YOU lead. Do you feel that you deserve to heal and feel better on your fertility journey? Do you feel like you deserve to be a mother and experience a healthier pregnancy? Do you feel like you deserve to grieve your losses and bring a baby home from the hospital? Of course, we say that we deserve these things, but the problem is that we battle an underlying feeling of unworthiness. In this episode: The qualifications we use to call ourselves “infertile” [3:20] How we, as women, allow everyone else’s needs to be more important than our own [12:20] When we don’t feel validated in our experience, we tend to look for validation from other people [17:00] Why we feel like we deserve the “bad things,” but don’t deserve healing and support [24:16] You don’t need to try to “figure out” if your pain is justified as compared with someone else’s [27:29] You shouldn’t have to fight for the ability to grieve, honor your story, and heal. You ARE worth it, and you deserve to heal from grief and trauma. When was the last time you told yourself that? November is Prematurity Awareness Month, so it’s a good time for us to discuss the basics. In today’s show, we’re talking about risk factors for preterm delivery and other information you need to know about this frightening experience. In this episode: Prematurity is the leading cause of death for children under five across the world---and the rate in the US keeps rising. [1:47] A history of preterm delivery is the #1 risk factor for another preterm delivery. [6:51] Many medical conditions and lifestyle habits can increase the chances of preterm delivery. [21:27] Conditions associated with stress can contribute to risk factors. [23:09] What you need to know about the four levels of the NICU--and what to expect with each one. [33:14] You need to be well-informed about the risk factors for preterm delivery, red flags to look for, who you need on your team, and how to prepare yourself. There are many nuances to grief. The loss of a child is extremely painful grief, but you have to trust that your body knows how to heal itself. It’s important that you reach out for support when you need professional guidance. Let’s jump into this difficult topic together. In this episode: We don’t speak accurately enough about the experience of pregnancy loss and saying goodbye to a life lost too soon. [9:28] Grief hurts and feels awful. [11:00] Our body has a built-in system that knows how to heal from grief. [19:55] There are no stages of grief. [25:07] Your job is to keep your body tension-free and tightness-free. [34:15] Grief is not a pathological condition. There is no treatment for it. You’re not broken for mourning the loss of your baby, so take all the time you need. One of the most interesting aspects of my work in working with a wide variety of people is that every experience is so different, even if they share the same starting point. Having a baby in the NICU is a different type of trauma that shifts what you understand to be normal. It shifts everything you know to be normal and everything you expect to encounter in the way you enter parenthood. The good news is that you CAN find yourself again and return to “normal”! Let’s talk about how to do it.…
September is NICU Awareness Month. If you aren’t familiar with what NICU stands for, it’s where sick or pre-term babies go for specialized care until they are well enough to (hopefully) go home. One of the most common questions I get from parents is about facing another pregnancy after going through a NICU experience. Just the thought of a repeat situation is terrifying for a parent. Today’s show dives into four important steps to take before trying to conceive again after a NICU experience. Episode Highlights: Talk to your partner Sometimes partners don’t feel included in the NICU experience Get your partner’s perspective and understand their feelings Make sure your partner isn’t stuck in the trauma response Realize that social support is essential for healthy pregnancy outcomes Work with a trauma-trained therapist if you need to Make a preconception appointment with a high-risk specialist Get a second opinion on what happened before and how to prevent it Get your questions answered before you try again Realize that this isn’t a one-time appointment, but may need to be repeated Think of this as selecting your team Deactivate your arousal system (turn off your stress response) Understand that none of this is your fault Know that a heightened stress response increases the chance of many pregnancy complications Look out for unresolved grief and trauma Know that mantras, affirmations, and positive thinking will NOT help Figure out when the time is right for you Don’t allow a medical professional to push you when you aren’t ready What can you make better right now? Realize that there is no right or wrong answer Trust your body; it won’t lie to you Resources: How to Get a Reliable /Second Opinion - http://bit.ly/003dmep What Should I Ask My Doctor Free Guide - bit.ly/whattoaskdr Get More Answers 30-Day Health Advocacy Program - bit.ly/getmoreanswers Path to Baby: a 1:1 Mind-Body Program to Prepare Your Body for Pregnancy - http://bit.ly/pathtobabydm Find me on Instagram: @healthy.highriskpregnancy…
Fertility trauma is happening all around us. It’s a huge part of the journey when you’re trying to conceive. Today’s show discusses why we have to address this issue and why trauma resolution should be part of fertility treatment for everyone. Episode Highlights: What is fertility trauma? Why trauma is NOT always based on a single event Examples of when and how fertility trauma may occur Why fertility trauma is not a mental health issue, but a physiological problem Health issues and inflammation that come when we don’t “come back down” from our body’s stress response Why you need to understand the physiology of what your body does---and how you can help it How trauma resolution can “stack the deck in your favor” Why stress is NOT the cause of infertility Why my clients are having fertility success when they put the puzzle pieces together Retraining your body with breath work, progressive relaxation, stretching, and dropping your shoulders The resulting changes that happen to the nervous, immune, and endocrine systems---all of which play a vital part in getting pregnant Resources: Optimize Your Fertility: http://bit.ly/optfertdm Path to Baby: http://bit.ly/pathtobabydm Instagram: healthy.highriskpregnancy…
I’ve talked to many women from many backgrounds with many complications. These women are all tied together by experiencing trauma on their family-building journey. The fact that unresolved trauma is a stress trigger fascinates me, and it led to my pursuit of training as a trauma specialist. Today’s show explores the question: What if the issues we see in maternal mental health are just results of unresolved trauma? We’re talking about common myths we hear regarding trauma healing and refuting them with truth. Episode Highlights: 5 Myths of Trauma Healing: You can only experience trauma after a life-threatening event Trauma occurs when the stress response is turned on and amped up without the opportunity to “come back down” Trauma is a life sentence, and it will be this way forever When we believe this myth, we forget the healing power of the human body Time heals all wounds The truth is that healing takes active participation PTSD is a disorder Post-traumatic stress is an injury and a wound You have to tell your story in order to heal Telling your story before you’re healed will lead to retraumatization The key to healing: You have to create the release, however that looks for you; relax your body and turn off the stress response Resources: www.parijatdeshpande.com/healinghearts Find me on Instagram: healthy@highriskpregnancy…
Some time ago, I came across a video on Facebook in which a pediatrician was showing a hold that soothes a crying newborn. One time when my daughter was crying, and nothing seemed to help, I remembered the instructional video and gave it a try. I was amazed at the results and became an instant believer in “The Hamilton Hold.” I wondered how I could spread this message that I believe every parent needs to hear, so I’m thrilled to introduce you to today’s guest! Dr. Robert Hamilton has written 7 Secrets of the Newborn: Secrets and (Happy) Surprises of the First Year , and has over three decades’ experience as a pediatrician. He’s the owner and founder of Pacific Ocean Pediatrics, established in 1996 in Santa Monica, CA. He is the creator of The Hamilton Hold, a method for soothing crying babies that has been seen over 30 million times on YouTube. In addition, Dr. Bob runs the LA marathon annually to support his medical mission organization which has been making annual trips to Africa and Central America for over 20 years. He’s also the proud father of six and grandpa to eight little ones. Dr. Bob lives in Santa Monica with his high school sweetheart, Leslie, to whom he has been married for 45 years.…
Why does saying NO feel so hard to do sometimes? The reason is that we want to be people-pleasers, well-liked, and the one who says YES to everything, but that’s just not possible in every situation. When I was on bed rest, I had to learn to say NO to many events and activities, and I felt like a terrible person. Maybe this is a lesson you’ve had to learn the hard way, too. Let’s dive into the subject with today’s guest! Brigid Dineen is here to teach us about her superpower: how to say NO without feeling like a jerk. Known for her deep empathy and grounded approach to personal growth, she is a resilience coach for strong women. Brigid teaches women how to say NO without feeling guilty, so they can reclaim their time, energy, and peace of mind. As the creator of Breathing Room and Mindfulness for Busy People, she provides practical tools and strategies for self-care. Brigid also hosts the podcast, Guts and Grace, which teaches how to create space for yourself in a world that would rather you stay small.…
Body image issues are common for girls during the adolescent and teen years, but do things improve at all for women in the postpartum? It’s exhausting and demoralizing to compare yourself to what you see on TV and in magazines--and know that your body does NOT look like what you see. She’s the author of Body Kindness: Never Say Diet Again and host of The Body Kindness podcast. Rebecca has a free anti-diet, get started course, and she provides one-on-one counseling, virtual support groups, workshops, e-courses, and more.…
The journey to becoming a parent may bring fertility challenges, stress, and fear of the unknown. Many changes take place, but your marriage can survive and thrive. Today’s show is about how to strengthen your relationship so you can get through the transition together.…
It’s another one of “those things” that no one tells you. After a high-risk pregnancy, difficult delivery, or lengthy NICU stay, the stress on your body creates the perfect environment for health problems and chronic illnesses. With everything else a new mom has to deal with after these difficulties, now she’s dealing with a mysterious illness! Many women have had this experience, which is insane, but probably more common than you think. Today’s guest shares her message of empowerment over the guilt, grief, and anger. Jen Hardy is an avid collector of eclectic diseases and mystifier of physicians! She’s an author, podcast host, and speaker who is married to her “Knight in Shining Armor” and is the mom to 7 children. Jen spent over two decades perfecting the art of managing a home while dealing with chronic illnesses, and she’s here to share what she’s learned.…

We all know how challenging and traumatic a stay in the NICU can be for new parents, but NICU isn’t the most common postpartum difficulty to be encountered. The postpartum journey is so different and so unexpected, that postpartum depression is actually the most common stressor. Most expectant parents think, “Oh, it won’t happen to me,” but the truth is that it’s more common than we realize. Let’s talk about how we can prepare ahead of time to minimize the chance of postpartum depression, and how a supportive partnership with your spouse or partner can prevent this from being your experience.…
The worst advice anyone can give a woman (or couple) going through infertility, fertility treatment or a high-risk pregnancy is, "You have no control". It is unhelpful and it is just plain wrong. Decades of research have shown the importance of believing in and maintaining control especially when it comes to health. Not only does it help you feel better emotionally, it creates physiological changes in the body, that during infertilty and a high-risk pregnancy, can increase your chances of bringing your baby home. On this episode, I share with you exactly how.…
"Don't give up." a comment I've heard all the time, through every stage of my journey and I'm sure many other women have heard as well. It's a comment that is meant to be kind and supportive, but it has really rubbed me the wrong way for a long time. Today I want to discuss why I don't like telling people "don't give up." and talk about real advice that can help someone who has lost that hope.…

It really is this hard. Let this sink in. Don't minimize your feelings. If it feels hard, it's okay. On this episode of Delivering Miracles, Parijat talks you through this important statement that every woman going through a high-risk pregnancy needs to hear.
The term "mind-body connection" can mean a variety of things to so many people. Some take it to mean mental health, others take it to mean nutrition therapy, others take it to mean something about the stars or hanging crystals by your bed. But most of us can agree that it relates somehow to stress. But does it have any impact on the health of your pregnancy? Absolutely. On this episode I share with you how.…
