It was understandable that much of the discussion in 2024 focused on how and where healthcare was falling short on cybersecurity. In general, healthcare had not prioritized or invested in cybersecurity compared to other regulated critical industries like banking and transportation. Healthcare IT Today thought it would be interesting to flip the script on cybersecurity in healthcare. Instead of focusing on where we needed to improve, we wanted to hear where we were doing well. PredictionHealth produces an ambient voice solution for physical therapy and occupational therapy settings, applying AI not just create the various post-visit notes they need but also to offer analytics. Pedro Teixeira points out some of the challenges that differentiate PT and OT from other clinical settings: sessions take place in gyms, the presence of music and clanging weights produces an "interesting audio environment" that the transcription tool must deal with, and people are often walking around, so some clinicans want to speak into a device clipped to their clothing and turn it off and on during a session. In this video, two hospital managers present an extended and intriguing view of major design changes that bring in state-of-the-art technology making sure that it "fades into the background" while clinical care takes the focus. Kara Tomlinson is Executive Director of System Care Delivery and Innovation and Ron Carson is Executive Director of Enterprise Applications at Nebraska Medicine. They have not let the age of their building—more than 50 years old—or the presence of legacy technologies keep them from dreaming big and "redefining what the hospital of the future means." One major example is replacing the dry erase board that is the focus of so many wards with a digital board from eVideon called Vibe Health. Although easy to use, the old boards limited efficiency and accuracy, were prone to human error, and—most important—weren't integrate with other systems. As a major differentiator from other AI-based ambient voice scribes, Health Note integrates pre-visit information into the note, shortening visits sometimes for 30 minutes. Pre-visit information is obtained from patients by sending them a text message several days before they show up for their visit. The information gathered is far more detailed and sophisticated than a standard intake, because it asks many questions a doctor would normally ask at the start of the visit. Through this pre-visit documentation, Health Note might write up to 90% of the doctor's note, according to Dr. Joshua Reischer, Co-Founder & CEO. The information typically includes the history of the present illness, a review of systems, demographics, social and family history, allergies, and more. Doctor note generation at Freed focuses on customization and personalization, to build "something to make clinicians happy." Indeed, comments posted by physicians online about the product are highly positive, such as "I got two hours of my day back." In pursuit of personalization—without adding complexity to the interface—Freed starts a generic ambient clinical note based on ChatGPT, then adds a post--processing step to adapt the tone and style of generated text to the medical specialty as well as to the individual physician's preferences. Jack Jeng, MD, Head of Clinical Affairs at Freed, says they started by targeting primary care, but have seen the product used for a wide variety of specialties, notably mental health, physical therapy, and occupational therapy. One of the most common questions we get from healthcare IT startups is how they can connect with EHR vendors. One of our most common questions we ask health IT startups is how they integrate with the EHR. One of the answers to this question for startup companies is FHIR. Certainly the FHIR standard is still maturing to include more data, but the data in the current FHIR standards can solve a lot of data access challenges for startup companies. If the data your startup needs is in FHIR, then the great thing is that pretty much every certified EHR can support FHIR access to that data (thanks EHR certification). One of the questions we get asked a lot by new digital health entrepreneurs here at Healthcare IT Today is whether building on top of FHIR will mean a solution will be interoperable. The answer, sadly, is no. Healthcare remains a mishmash of data formats. Andrea Kowalski, Chief Product Officer at 1upHealth, shares this pragmatic view on the state of interoperability. Prior Authorization is a not a popular topic. In fact, we have not found many organizations willing to talk about it. Simply put, it is a complex challenge. We were pleasantly surprised when Frank Harvey, the CEO of Surescripts was willing to come on-camera to share the work they are doing in this area. Surecripts is collaborating with Cleveland Clinic and OptumRx to streamline the prior authorization process. What used to take days or hours can now be completed in seconds – without changing physician workflows. This is possible through careful coordination of data between the provider, payer, and Surescripts. This advancement not only ensures patients can access critical therapies sooner, but also reduces provider burnout. For now, this process is only for a limited number of therapeutic areas, but Surescripts plans to expand to more in the coming year.
